Commandant’s Final Action Memorandum on the Report of the Investigation into the Circumstances Surrounding the Passenger Deaths while on an excursion vessel launched from the Foreign Cruise Ship World Explorer

The Coast Guard’s Office of Investigations and Casualty Analysis (CG-INV) recently posted the Commandant’s Final Action Memorandum (FAM) detailing the Coast Guard’s responses and actions on each recommendation proposed in the Report of Investigation (ROI) regarding the capsizing of an inflatable Zodiac excursion vessel launched from the foreign cruise ship WORLD EXPLORER, while on a sightseeing tour off the coast of Elephant Island, Antarctica, which resulted in the loss of two U.S. passenger lives on Nov. 15th, 2022. This investigation was conducted by Activities Europe, U.S. Coast Guard, in cooperation with Portuguese Maritime Accident Investigation and Aeronautical Meteorology Authority Office (GAMA), Argentina Marine, Inland Water and Lakes Investigation (JST), and National Transportation Safety Bureau (NTSB). As a result of this investigation, two Safety Recommendations and two Administrative Recommendations were issued. In addition, GAMA investigation report dated May 2nd, 2023, resulted in one Safety Recommendation being issued.
